Immediately after the publication of Darwin's book on the origin of species, Engels, having familiarized himself with it, wrote to Marx on December 12, 1859: "All in all, Darwin, whom I am reading right now, is superb. Teleology had hitherto not yet been destroyed in one of its aspects, and now this has been done. Moreover, hitherto there has never been so sweeping an attempt to prove historical development in nature, especially with such success." Soon Marx enlarged upon the evaluation (...) of Darwinism. In his letter of December 19, 1860, he commented that this theory "provides a foundation in natural history for our views." It is therefore no accident that Engels, in developing the principles of dialectical materialism and problems of the dialectics of nature, interested himself so deeply and comprehensively in the theory of evolution. Background: Hospital nurses are frequently the first care givers to receive a patient’s request for euthanasia or physician-assisted suicide (PAS). In France, there is no consensus over which medical practices should be considered euthanasia, and this lack of consensus blurred the debate about euthanasia and PAS legalisation. This study aimed to investigate French hospital nurses’ opinions towards both legalisations, including personal conceptions of euthanasia and working conditions and organisation. Methods: A phone survey conducted among a random national sample of 1502 (...) French hospital nurses. We studied factors associated with opinions towards euthanasia and PAS, including contextual factors related to hospital units with random-effects logistic models. Results: Overall, 48% of nurses supported legalisation of euthanasia and 29%, of PAS. Religiosity, training in pallative care/pain management and feeling competent in end-of-life care were negatively correlated with support for legalisation of both euthanasia and PAS, while nurses working at night were more prone to support legalisation of both. The support for legalisation of euthanasia and PAS was also weaker in pain treatment/palliative care and intensive care units, and it was stronger in units not benefiting from interventions of charity/religious workers and in units with more nurses. Conclusions: Many French hospital nurses uphold the legalisation of euthanasia and PAS, but these nurses may be the least likely to perform what proponents of legalisation call “good” euthanasia. Improving professional knowledge of palliative care could improve the management of end-of-life situations and help to clarify the debate over euthanasia. (shrink)

BackgroundThe COVID-19 pandemic has created ethical challenges for intensive care unit (ICU) professionals, potentially causing moral distress. This study explored the levels and causes of moral distress and the ethical climate in Dutch ICUs during COVID-19.MethodsAn extended version of the Measurement of Moral Distress for Healthcare Professionals (MMD-HP) and Ethical Decision Making Climate Questionnaire (EDMCQ) were online distributed among all 84 ICUs. Moral distress scores in nurses and intensivists were compared with the historical control group one year before COVID-19. ResultsThree (...) hundred forty-five nurses (70.7%), 40 intensivists (8.2%), and 103 supporting staff (21.1%) completed the survey. Moral distress levels were higher for nurses than supporting staff. Moral distress levels in intensivists did not differ significantly from those of nurses and supporting staff. “Inadequate emotional support for patients and their families” was the highest-ranked cause of moral distress for all groups of professionals. Of all factors, all professions rated the ethical climate most positively regarding the culture of mutual respect, ethical awareness and support. “Culture of not avoiding end-of-life-decisions” and “Self-reflective and empowering leadership” received the lowest mean scores. Moral distress scores during COVID-19 were significantly lower for ICU nurses (p < 0.001) and intensivists (p < 0.05) compared to one year prior.ConclusionLevels and causes of moral distress vary between ICU professionals and differ from the historical control group. Targeted interventions that address moral distress during a crisis are desirable to improve the mental health and retention of ICU professionals and the quality of patient care. (shrink)

ABSTRACT Vague concepts are represented by L-fuzzy sets. It is argued that any vague concept carries with it an approximate identity which is a fuzzy equivalence relation. The relation also fulfills the criterion of ? indiscernibility of Identicals ?, which is called ? saturatedness ? in this context. An application in knowledge representation is indicated.

Merab Konstantinovich [Mamardashvili] met with me immediately, as soon as I requested it, although he forewarned me that he could only dimly remember much of that distant past in which I was most interested. But evidently that past still perturbed him as well, since he agreed to speak with me even though he had not yet completely recovered from his illness, and hence his voice was feeble, at times subsiding to a whisper; he would pronounce his words indistinctly, constantly sticking (...) one onto another; on top of that, for technical reasons the dictaphone was situated a bit away from him. It was therefore very difficult to transcribe the text, and I raised many questions with the idea of asking Merab Konstantinovich about them at our next meeting and clearing up some things. But alas, there was no next meeting … Hence I am permitting myself now to publish only some of the most clearly enunciated fragments of my conversation with him, one of the four "forefathers" of the Moscow Methodological Circle, whose name for us, today's participants (and hence neophytes) in the methodological movement, is enveloped by legends handed down by word of mouth. I kept my questions to a minimum. Mamardashvili's reminiscences, arguments, and reflections will seem contentious to many. But no matter, one can dispute points of substance even with those who have passed on. (shrink)

Clinical ethics support, including ethics consultation, has become established in the field of medical practice throughout the world. This practice has been regarded as useful, most notably in the UK and the USA, in solving ethical problems encountered by both medical practitioners and those who receive medical treatment. In Japan, however, few services are available to respond to everyday clinical ethical issues, although a variety of difficult ethical problems arise daily in the medical field: termination of life support, euthanasia and (...) questions about patient autonomy. In light of these conditions, a group of 17 volunteer educators and researchers from the area of biomedical ethics, including the authors, have formed the Clinical Ethics Support and Education Project, and began providing Japan’s first small team clinical ethics consultation service in October, 2006. Members include scholars of biomedical ethics, scholars of philosophy and ethics, legal professionals and legal scholars, nurses and doctors, consisting of five women and 12 men. Consultation teams, made up of a small number of members, were organised each time a request for consultation was received. Over approximately 15 months (October 2006–December 2007), the programme received 22 consultation requests from medical practitioners and medical institutions, and three from the families of patients. In this paper, we will discuss the status of our consultation service and examples of consultation cases we have handled. In addition, we will examine the process of evaluating small team clinical ethics consultation services, as well as the strengths and weakness of such programmes. (shrink)

An attempt by one of the leading political scientists to "extricate from the total political reality those aspects that can be considered the fundamental processes or activities without which no political life in society could continue." The conceptual framework for a "more complex structure of a theory" is developed around the concepts of demand, support, stress, and input-output. Easton explicitly rejects the discussion of political theory in terms of what he calls political philosophy, but the work is nevertheless of interest (...) to philosophers who have the stamina and interest to wade through a long and over-drawn discussion of the nature of political life.—K. Background One of the objectives of medicine is to relieve patients' suffering. As a consequence, it is important to understand patients' perspectives of suffering and their ability to cope. However, there is poor insight into what determines their suffering and their ability to bear it. Purpose To explore the constituent elements of suffering of patients who explicitly request euthanasia or physician-assisted suicide (EAS) and to better understand unbearable suffering from the patients' perspective. Patients and methods A qualitative study using in-depth (...) face-to-face interviews was conducted with 31 patients who had requested EAS. The grounded theory approach was used to analyse the data. Results Medical, psycho-emotional, socio-environmental and existential themes contributed to suffering. Especially fatigue, pain, decline, negative feelings, loss of self, fear of future suffering, dependency, loss of autonomy, being worn out, being a burden, loneliness, loss of all that makes life worth living, hopelessness, pointlessness and being tired of living were constituent elements of unbearable suffering. Only patients with a psychiatric (co)diagnosis suffered unbearably all the time. Conclusions Unbearable suffering is the outcome of an intensive process that originates in the symptoms of illness and/or ageing. According to patients, hopelessness is an essential element of unbearable suffering. Medical and social elements may cause suffering, but especially when accompanied by psycho-emotional and existential problems suffering will become ‘unbearable’. Personality characteristics and biographical aspects greatly influence the burden of suffering. Unbearable suffering can only be understood in the continuum of the patients' perspectives of the past, the present and expectations of the future. (shrink)

Sixteen authors ranging from Plato to Gandhi are used to provide a "comprehensive source book in the area of social and political philosophy." About forty pages of texts are provided from each author, but many of the selections have been edited extensively. Although there are no selections from the period between Aristotle and Machiavelli, more recent figures, such as Thoreau, Engels, Mussolini and Hitler are included.--K. A. M.

A practical book for teaching beginning speech is attached to a confusing essay in which rhetoric is said to be the means by which "Man the Talker" can overcome the "crisis in values." Lasswell's analysis of values is said to form the basis of this text book, but what relation the essay on values has to the practical speech book is not made clear.—K. A. M.

Stewart presents a very useful and worthwhile exposition of a part of Hume's philosophy which is often overlooked. Even if one does not share the author's view that Hume "was primarily a political and moral theorist," a reading of this work will show that Hume has much of interest of say in the area. Of particular service is the concise presentation of the more relevant parts of Hume's eight volume History of England. Stewart is a practicing politician and his book (...) clearly demonstrates that a great philosopher such as Hume affords access from many directions, and that philosophic understanding can be enriched by contributions from many sources.—K. An enjoyable and well-written discussion of the change in the conception of politics from David Hume through Jeremy Bentham, and John Stuart Mill to Beatrice Webb. The longest, most interesting, and most useful section is on Hume, who expresses the dominant view of the eighteenth century that "man is a balanced whole whose object is to live decently and enjoyably." The discussion of Hume is the background for a less favorable consideration of Bentham, Mill, and Webb and although the latter (...) half of the book fails to match the first half in depth and interest, it remains a work which can be recommended and read with joy.—K. A collection of articles in English, German and French by fourteen Hungarian authors. Of particular interest and value is "Über die erkenntnistheoretischen Ansichten des jungen Marx" by György Markus. Although the footnotes are full of criticisms of the "revisionists", the early Marx is taken seriously as a philosophical thinker in his own right, and an effort is made to lay out what might be a theory of knowledge for this early Marx. Other articles of interest are by Edit Rózsahegyi, "The (...) Place and Role of Purpose in the Process of Cognition" and a criticism of Heisenberg by A. Szabó. On the whole, the articles are genuinely philosophical and deserve attention, although long, boring and dogmatic pieces such as "Objective Contradiction" by G. Tamás, and "Communism and Conditions for the Development of Personality" by A. Wirth are also present.--K. A. M. (shrink)

Modern microscopic theory is employed to construct a powerful analytical algorithm that permits a clear description of characteristic features of strongly correlated quantum fluids in thermodynamic equilibrium. Using recently developed formal results we uncover an intricate relationship between strongly correlated systems and free quantum gases of appropriately defined constituents. The latter entities are precisely defined renormalized bosons or fermions. They carry all the information contained in the statistical correlations of the strongly interacting many-particle system by virtue of their effective masses. (...) The mass of such a renormalized boson or fermion depends in a specific form on temperature, bulk particle number density of the many-body system, and eventually on momentum. Due to these properties the renormalized bosons and fermions determine in particular the location and nature of the transition to non-normal phases. (shrink)

A new translation of five dialogues of Bruno translated in 1950 by Sidney Greenberg in The Infinite in Giordano Bruno under the title "Concerning the Cause, Principle and One." A rather superficial introduction fails to substantiate the claim that the "materialistic dialectic" is the core of Bruno. It is not clear why a new translation is needed, except for the fact that Greenberg's book is unfortunately out of print.—K. A. M.

A scholarly account of an important and previously uninvestigated aspect of Bruno's philosophy. Yates sets out in the historian's careful way to show that "Bruno's philosophy and his religion are one and the same, and both are Hermetic." A treatment of the development of the Hermetic tradition from Ficino and Pico allows the author to show that "the philosophy of the infinite universe and the innumerable worlds... is not... scientific thinking" but a continuation of the tradition which was mistakenly thought (...) to stem from an ancient Egyptian magical religion. Although this is definitely the work of an historian, any philosopher interested in the beginnings of modern science or in Renaissance thought will find it a valuable aid and contribution.—K. A. M. A thorough historical account of the unsuccessful attempt by the Philosophic Radicals to establish an independent political party in the 1830's. Although definitely an historical, rather than a philosophical treatment of the early followers of John Mill, it does give a great deal of detailed information on a relatively unknown and unsuccessful period of J. Although Ash does put the Marxist position in language familiar to the English reader, both Marxism and moral concepts are not treated in depth. Marxism is primarily a theory which holds values are based in a direct way on economic relations. Recent advances in Marx scholarship or discussions with the Marxist movement are ignored as the attack is focused on the capitalist order.—K. A. M.

Greenleaf combines solid historical scholarship with philosophical understanding in this useful and detailed study of political thought in 16th and 17th century England. The first half of the book reconstructs the position of the royalists and argues that the philosophical defense of absolute monarchy is much stronger than Locke would lead us to believe in his attack on Filmer. After a chapter on Bodin the empiricists are discussed as the forerunners of empirical and historical study of politics. The concluding chapter (...) summarizes the discussion and establishes the background for Locke's political writings.—K. A. M. (shrink)
